1. Full name as it appears in your Stevens records
2. Stevens ID number
3. Mailing address
4. Home phone numbers and hours you may be reached
5. Business phone number, if any, and hours you may be reached
6. E-mail address(es); send multiple addresses if you have more than one
7. Name of faculty member supervising your oral exam.
